Community Immunizations

School Immunization Requirements

Immunizations are required by law for children in attendance at public and private schools, preschools, childcare facilities and Head Start programs in Oregon. Nearly every place that provides care for a child outside the home requires immunizations or a religious or medical exemption to stay enrolled.

Children who are not up-to-date on their immunizations by February 17, 2010 will be excluded from school. See required immunizations, below.


Important Dates 2009-2010

First Day of School

All children must show proof of one dose of all required immunizations.
EXCEPTION: Students who transfer from another school in the United States have one month to have their immunization records transferred to the new school.

January 13, 2010

Deadline for schools to report to Multnomah County Health Department.

February 3, 2010

Multnomah County Health Department mails exclusion orders.

February 17, 2010

Exclusion Day. All children who are not up-to-date or complete on their immunizations will be excluded from school.

Parent Responsibility

Parents should print a copy of the Oregon Certificate of Immunization Status form. Fill in the shot dates for your child, sign the form and turn it into your child’s school. Your child must show proof of one dose of each of the required immunizations by the first day of school.

School and Childcare Facility Responsibility

Schools and childcare facilities need to have one Oregon Certificate of Immunization Status form or ALERT record on file for each child at the school. Schools report incomplete or non-existing shot records to the Multnomah County Health Department in January.

Health Department Responsibility

The Community Immunization Program reviews all records sent to us by childcare facilities, preschools, Head Starts and kindergartens. We issue exclusion orders to children who are not up-to-date on their immunizations. Children who are not up-to-date on their immunizations by February 17, 2010 will be excluded from school.

*Doses required varies by a child’s age and how long ago they were vaccinated. Please check with your child’s school or healthcare provider for details.

What should I do if I receive an exclusion notice?
Contact your medical provider to have your child immunized. To stay enrolled, your child must meet the current immunization requirements by February 17, 2010. See timeline, above.
